REQUIREMENTS TO PROVIDE A 232 LOAN ANALYSIS FOR NEW CONSTRUCTION OR SUBSTANTIAL RENOVATION OF
NURSING HOMES & ASSISTED LIVING FACILITIES

 

If you provide AMERICAN MORTGAGE with the following preliminary information, we will be pleased to provide you with a mortgage proposal which will outline the level, terms, and conditions of the financing we can arrange for you:

  1. Project description.
  2. Site location map.
  3. Current and/or anticipated daily rates and patient mix.
  4. Annual operating statement for past 3 years and year to date (as applicable).
  5. If refinance, summary of existing debt and copy of mortgage(s) and note(s).
  6. If acquisition; copy of option or purchase contract.
  7. Construction budget (or) list of proposed repairs and cost estimate.
  8. Resume on proposed sponsor, management agent, and architect and contractor (if selected.).
  9. Market study or appraisal (if available).
  10. Survey, site plan, or architectural drawings (if completed).
  11. Copy of Certificate-Of-Need and/or Nursing Home License (if applicable).
  12. Lease or Management Agreement (if applicable).
  13. Post Construction bed mix including unit sizes, number of bedrooms, number of bathrooms, and proposed bed rates.
  14. Post Construction operating expenses (detailed).
  15. List of any proposed ancillary income detailing the source and anticipated monthly revenue.
  16. Current census. (Rehab projects)
  17. Occupancy history for most recent year and each of the last six months. (Rehab projects)
  18. Most recent Department of Health Inspection report, if applicable. (Rehab projects)
